This is a combination therapy comprising four distinct antineoplastic agents: panitumumab, gemcitabine, oxaliplatin, and capecitabine. Panitumumab (brand name Vectibix) is a fully human monoclonal antibody that targets the epidermal growth factor receptor (EGFR), preventing its activation and subsequent downstream signaling, thereby inhibiting cell proliferation, survival, and angiogenesis, and mediating antibody-dependent cellular cytotoxicity (ADCC). Gemcitabine (brand name Gemzar) is a nucleoside analog prodrug that, once metabolized, incorporates into DNA to cause masked chain termination and inhibits ribonucleotide reductase, ultimately disrupting DNA synthesis and inducing cell death. Oxaliplatin (brand name Eloxatin) is a platinum-based antineoplastic agent that forms DNA adducts, leading to inter- and intra-strand crosslinks that impede DNA replication and transcription, resulting in cell cycle arrest and apoptosis. Capecitabine (brand name Xeloda) is an oral prodrug of 5-fluorouracil (5-FU), an antimetabolite that inhibits thymidylate synthase and incorporates into RNA and DNA, thereby disrupting nucleic acid synthesis and function. This multi-pronged approach aims to target various pathways critical for cancer cell growth and survival. While each component is approved for various cancers, the specific combination of all four drugs would typically be investigated for advanced or refractory malignancies, particularly those of gastrointestinal origin, leveraging their complementary mechanisms of action.
