The regimen is structured into two distinct phases: the first 5 days involve the administration of a proton pump inhibitor (pantoprazole 40 mg twice daily) and amoxicillin (1 g twice daily); the subsequent 5 days consist of the proton pump inhibitor (pantoprazole 40 mg twice daily) combined with clarithromycin (500 mg twice daily) and metronidazole (500 mg twice daily). This sequential approach is designed to improve eradication rates by first reducing the bacterial load and weakening the cell wall with amoxicillin, thereby enhancing the efficacy of clarithromycin and metronidazole in the second phase, particularly in populations with high antibiotic resistance.
