Papacarie-Duo is a chemo-mechanical caries removal (CMCR) gel primarily used in minimally invasive dentistry to remove decayed dentin while preserving healthy tooth structure. The formulation is a combination of papain, a proteolytic enzyme extracted from the papaya fruit (Carica papaya), and chloramine. Its mechanism of action involves the enzymatic degradation of necrotic, denatured collagen found in carious dentin. Unlike healthy dentin, which contains alpha-1-antitrypsin to prevent proteolysis, carious dentin lacks this protection, allowing the papain to specifically break down the damaged tissue. Chloramine acts as a disinfectant and helps to soften the carious dentin by pre-treating the collagen fibers. This treatment significantly reduces the need for high-speed drills and local anesthesia, making it an effective alternative for pediatric patients, dental phobics, and pregnant women where traditional interventions might be avoided or delayed.
