Paquinimod is a synthetic small molecule immunomodulatory drug developed primarily for the treatment of autoimmune diseases such as systemic lupus erythematosus (SLE) and systemic sclerosis. It is a quinoline derivative that acts as an inhibitor of the S100A9 protein, thereby preventing its interaction with Toll-like receptor 4 (TLR4). This mechanism modulates inflammatory responses by reducing pro-inflammatory cytokine production and fibrotic processes. Paquinimod has been studied in clinical trials for SLE, systemic sclerosis, and experimental osteoarthritis. The drug was granted orphan drug designation by the US FDA for systemic scleroderma but development has since been discontinued for all major indications[1][2][3][4][6].
