This combination therapy involves the co-administration of paracetamol (acetaminophen) and dexamethasone for the management of acute renal colic pain. Paracetamol is a non-opioid analgesic and antipyretic that acts primarily by inhibiting prostaglandin synthesis in the central nervous system. Dexamethasone is a potent, long-acting synthetic glucocorticoid with significant anti-inflammatory and immunosuppressive properties. In a clinical trial conducted by the University of Monastir, intravenous dexamethasone (8mg) is being evaluated as an adjuvant to intravenous paracetamol (1000mg) to determine if it provides superior pain relief compared to paracetamol alone or paracetamol combined with nebulized salbutamol. The rationale for using dexamethasone in renal colic is to reduce the inflammatory response and ureteral edema associated with the passage of urinary stones, thereby potentially enhancing the analgesic effect of paracetamol.
