Drug intelligence / Profile preview
paracetamol + morphine + tramadol + ketamine is a multimodal analgesic combination regimen being evaluated for the management of acute pain during vaso-occlusive crises (VOC) in adult patients with sickle cell disease. This combination leverages multiple distinct mechanisms of action to optimize pain control while potentially reducing overall opioid consumption and associated adverse effects. Paracetamol (acetaminophen) acts as a weak inhibitor of cyclooxygenase (COX-1 and COX-2) enzymes to provide antipyretic and analgesic effects. Morphine is a potent mu-opioid receptor agonist that provides strong central analgesia. Tramadol acts as both a weak mu-opioid receptor agonist and a serotonin-norepinephrine reuptake inhibitor (SNRI), enhancing inhibitory pain pathways. Ketamine is a non-competitive N-methyl-D-aspartate (NMDA) receptor antagonist that provides additional co-analgesia and helps prevent opioid-induced hyperalgesia. The combination is being studied in the PAMAVOC adaptive platform trial sponsored by Assistance Publique - Hôpitaux de Paris.
