Paracetamol + piroxicam is an investigational combination of the aniline analgesic paracetamol (acetaminophen) and the nonsteroidal anti-inflammatory drug (NSAID) piroxicam. It has been primarily studied by the University of Monastir in Tunisia for the management of acute post-traumatic pain in the extremities. Paracetamol is believed to exert its analgesic and antipyretic effects through the inhibition of prostaglandin synthesis within the central nervous system. Piroxicam, an oxicam derivative, acts as a non-selective inhibitor of the cyclooxygenase enzymes COX-1 and COX-2, thereby reducing the production of prostaglandins that mediate pain and inflammation both peripherally and centrally. In a Phase 3 clinical trial, the co-administration of these two agents was compared against monotherapy with each drug; the study concluded that the combination did not provide a statistically significant increase in analgesic efficacy over paracetamol alone for acute traumatic extremity pain.
