Parathyroid hormone 1-84 is a full-length recombinant form of human parathyroid hormone (PTH), used as an adjunct to calcium and vitamin D to control hypocalcemia in patients with hypoparathyroidism. It is administered as a once-daily subcutaneous injection. The drug acts by mimicking endogenous PTH, regulating serum calcium through effects on bone resorption, renal calcium reabsorption, and indirectly increasing intestinal calcium absorption via stimulation of active vitamin D synthesis. This results in increased serum calcium from bone, kidney, and intestine without altering serum phosphate concentration. Parathyroid hormone 1-84 was the first approved PTH replacement therapy for chronic hypoparathyroidism not adequately controlled with standard therapy.
