Parecoxib + oxycodone + ropivacaine is a multimodal analgesic (MMA) regimen being evaluated for the management of postoperative pain following gynecological surgeries, specifically laparoscopic hysterectomy for benign uterine diseases or early uterine malignancies. This combination therapy, investigated by Xuanwu Hospital of Capital Medical University, utilizes three distinct mechanisms to optimize pain control: parecoxib, a selective cyclooxygenase-2 (COX-2) inhibitor, reduces peripheral inflammation and prostaglandin synthesis; oxycodone, a semi-synthetic opioid, acts as a mu-opioid receptor agonist to modulate central pain perception; and ropivacaine, a long-acting amino-amide local anesthetic, blocks voltage-gated sodium channels to inhibit local nerve conduction. By combining these agents, the regimen aims to provide superior analgesia, facilitate earlier mobilization, and reduce the total consumption of opioids, thereby minimizing associated side effects such as nausea, vomiting, and respiratory depression.
