Parecoxib + valdecoxib

Parecoxib + valdecoxib is a sequential combination therapy involving two closely related nonsteroidal anti-inflammatory drugs (NSAIDs). Parecoxib is an injectable, water-soluble prodrug that is rapidly converted in vivo to valdecoxib, its active form. Both drugs are selective cyclooxygenase-2 (COX-2) inhibitors, providing anti-inflammatory, analgesic, and antipyretic effects primarily through inhibition of COX-2-mediated prostaglandin synthesis. This combination has been used for the short-term management of acute postoperative pain when oral administration is not feasible. Parecoxib allows for perioperative intravenous administration; after initial treatment with parecoxib injection, patients may be transitioned to oral valdecoxib for continued pain control. The primary developer and manufacturer was Pfizer. Valdecoxib and its prodrug parecoxib have been associated with increased cardiovascular risk in certain populations (notably post-CABG surgery), leading to regulatory restrictions or withdrawal from some markets[1][2][5].
