PAS-IL-1Ra is a long-acting recombinant fusion protein consisting of the human interleukin-1 receptor antagonist (IL-1Ra) and a PASylation tail. Developed by XL-protein GmbH, the drug utilizes the proprietary PASylation technology—a genetic fusion with a conformationally disordered polypeptide sequence composed of Proline, Alanine, and Serine (PAS)—to increase the hydrodynamic volume of the molecule. This modification significantly extends the plasma half-life by reducing renal filtration, addressing the rapid clearance and short half-life characteristic of the native IL-1Ra (anakinra). PAS-IL-1Ra acts as a competitive inhibitor of interleukin-1 (IL-1) by binding to the IL-1 receptor type I (IL-1RI), thereby blocking the pro-inflammatory signaling of both IL-1α and IL-1β. It is primarily investigated for the treatment of chronic inflammatory conditions such as rheumatoid arthritis and gout, where a reduced dosing frequency compared to daily anakinra injections would be therapeutically advantageous.
