This combination consists of donor human milk that has been pasteurized (typically via Holder pasteurization at 62.5°C for 30 minutes) and subsequently enriched with a human milk fortifier (HMF). HMFs are concentrated nutritional supplements containing additional protein, carbohydrates, fats, vitamins, and minerals (such as calcium and phosphorus) to meet the high metabolic demands of preterm or very-low-birth-weight (VLBW) infants. When both the base milk and the fortifier are derived from human milk, the regimen is clinically referred to as an exclusive human milk diet (EHMD). This approach is primarily used in neonatal intensive care units (NICUs) to promote growth and bone mineralization while providing bioactive components like human milk oligosaccharides (HMOs), lactoferrin, and immunoglobulins that support gut maturation and immune development. Clinical evidence suggests that an EHMD significantly reduces the risk of necrotizing enterocolitis (NEC) and other prematurity-related morbidities compared to bovine-based fortification.
