Overview
The "patch protocol" is a specific controlled ovarian stimulation regimen used in assisted reproductive technology (ART), particularly for women identified as "poor responders" or those with poor ovarian reserve. As implemented by the Ottawa Fertility Centre, the protocol involves the administration of a transdermal estrogen patch (estradiol) starting in the luteal phase of the preceding cycle, followed by a GnRH antagonist (cetrorelix) to prevent premature ovulation, and high-dose gonadotropin stimulation using recombinant follicle-stimulating hormone (r-FSH) and recombinant luteinizing hormone (r-LH). The cycle is completed with a human chorionic gonadotropin (hCG) trigger for oocyte maturation and luteal phase support with vaginal progesterone. This approach aims to synchronize follicle development and improve oocyte yield in patients who have previously failed to respond adequately to standard IVF protocols.
