Overview

Patiromer is an oral, non-absorbed, cation exchange polymer used to treat hyperkalemia (high blood potassium). It acts as a potassium binder by exchanging calcium for potassium ions in the gastrointestinal tract, primarily in the colon. This binding reduces free potassium available for absorption and increases fecal excretion of potassium, thereby lowering serum potassium levels. Patiromer is not systemically absorbed or metabolized and is excreted unchanged in the feces. It is indicated for chronic management of hyperkalemia but should not be used for emergency treatment of life-threatening cases due to its delayed onset of action. The drug was developed to address hyperkalemia particularly in patients with chronic kidney disease or those on renin-angiotensin-aldosterone system inhibitors[1][2][3][6][7].
