Pb-203-RMX-VH (also known as 203Pb-RMX-VH-PIB) is an investigational radiopharmaceutical companion diagnostic developed by RadioMedix in collaboration with Vect-Horus. It consists of a DOTAM-modified peptide targeting vector (RMX-VH) that binds with high affinity to the low-density lipoprotein receptor (LDLR), which is overexpressed in various solid tumors, including glioblastoma multiforme (GBM) and pancreatic ductal adenocarcinoma (PDAC). The peptide is radiolabeled with lead-203 (Pb-203), a gamma-emitting radionuclide suitable for SPECT/CT imaging. This imaging agent is designed to cross the blood-brain barrier to evaluate biodistribution, dosimetry, and tumor targeting efficiency, serving as a companion diagnostic to select patients for targeted alpha therapy with its therapeutic counterpart, Pb-212-RMX-VH. It is currently in Phase 1 clinical development.
