PBI-4547 is a novel, orally active small molecule developed as a fatty acid mimetic for the treatment of metabolic diseases. It acts as an antagonist of G protein-coupled receptor 84 (GPR84) and as an agonist of GPR40 (Free fatty acid receptor 1) and GPR120 (Free fatty acid receptor 4), with partial activation of peroxisome proliferator-activated receptors. Its mechanism involves stimulating fatty acid oxidation, inducing gene expression of mitochondrial uncoupling proteins in the liver, and improving metabolic dysregulation associated with high-fat diet regimens. Preclinical studies have shown that PBI-4547 improves parameters related to non-alcoholic fatty liver disease (NAFLD), diabetes, obesity, hepatic fibrosis, and metabolic syndrome by reducing hepatic steatosis, inflammation, fibrosis markers in liver and kidney tissues, lowering blood glucose and lipid levels, increasing adiponectin levels, promoting browning of white adipose tissue, and preventing renal structure destruction in diabetic models[1][2][3][4][5][6].
