PCART7 is an autologous chimeric antigen receptor (CAR) T-cell therapy designed to target CD7-positive hematologic malignancies, such as relapsed or refractory (r/r) acute myeloid leukemia (AML) and T-cell acute lymphoblastic leukemia (T-ALL). Developed using technology from MediSix Therapeutics, the therapy utilizes a lentiviral vector to express a 4-1BB-based CAR specific for CD7. A distinguishing feature of PCART7 is the incorporation of a protein expression blocker (PEBL), which prevents the surface expression of CD7 on the therapeutic T cells themselves. This modification is critical to prevent 'fratricide,' where CAR-T cells target and kill each other due to the natural expression of CD7 on T cells. By sequestering endogenous CD7 within the cell, the PEBL technology enables the successful manufacture, expansion, and clinical persistence of the CAR-T cells. PCART7 has demonstrated the ability to induce deep molecular remissions in patients with high CD7-expressing leukemia, often serving as a bridge to allogeneic hematopoietic stem cell transplantation.
