PCO‑371 is a non-peptide small molecule that acts as a full agonist of the parathyroid hormone receptor type 1 (PTH1R), a class B G protein-coupled receptor. It was developed by Chugai Pharmaceutical for the potential treatment of hypoparathyroidism and other parathyroid hormone (PTH)-related disorders. Unlike peptide-based therapies such as recombinant human PTH(1–34), PCO‑371 is orally bioavailable and selectively activates human PTHR1 without significant activity at the related type 2 receptor. Preclinical studies demonstrated its ability to increase bone mineral density and restore serum calcium in animal models of osteopenia and hypocalcemia. The drug entered phase I clinical trials in healthy volunteers but development has since been discontinued[3][7][5][6].
