PCSK9 inhibitor + statin refers to the combination therapy of a proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor with a statin. This regimen is used for patients with hyperlipidemia or high cholesterol, particularly those at high risk for cardiovascular events who do not achieve target LDL cholesterol levels on maximally tolerated statins alone. PCSK9 inhibitors, such as evolocumab and alirocumab, are monoclonal antibodies that block the PCSK9 protein, increasing the number of LDL receptors available to clear LDL cholesterol from the blood. Statins inhibit HMG-CoA reductase in the liver, reducing endogenous cholesterol synthesis and upregulating LDL receptors. The combination results in additive or synergistic lowering of LDL-C levels and further reduces cardiovascular risk compared to either agent alone[6][7]. This approach is especially beneficial for patients with familial hypercholesterolemia or established atherosclerotic cardiovascular disease who require intensive lipid-lowering therapy[5][6].
