PCSK9Qβ-003 is a therapeutic vaccine candidate targeting proprotein convertase subtilisin/kexin type 9 (PCSK9). It is formulated as a peptide conjugated to Qβ bacteriophage virus-like particles (VLPs), designed to elicit antibody responses against PCSK9, thereby inhibiting its function. Mechanistically, this vaccine lowers total cholesterol and low-density lipoprotein cholesterol (LDL-C) by blocking PCSK9, which inhibits PCSK9-mediated degradation of the LDL receptor (LDLR), resulting in increased LDLR levels and enhanced clearance of LDL-C. Preclinical studies demonstrated that PCSK9Qβ-003 reduces atherosclerotic lesion area, promotes plaque stability by enhancing reverse cholesterol transport, upregulates liver X receptor alpha and ATP-binding cassette transporter A1, suppresses inflammation, and decreases apoptosis within the aorta. It has also shown benefit in mouse models of renal fibrosis, improving lipid accumulation and fibrosis by modulating fatty acid β-oxidation and upregulating relevant hepatic and renal lipid metabolism regulators. Developed as a potential cost-effective alternative or adjunct to statins for the treatment of atherosclerosis, hypercholesterolemia, and related organ damage, especially in statin-intolerant or resistant patients[1][2][3][5][7].
