Overview

PD-1 inhibitor + PCSK9 inhibitor refers to the combination of two therapeutic modalities: an anti-PD-1 immune checkpoint inhibitor (such as cemiplimab or sintilimab) and a PCSK9 inhibitor (such as alirocumab or tafolecimab). This combination is being explored as an experimental cancer immunotherapy. PD-1 inhibitors block the programmed cell death protein 1 (PD-1) pathway to restore T cell-mediated antitumor immunity. PCSK9 inhibitors, originally developed to lower cholesterol, have been found to enhance the efficacy of PD-1 inhibitors by increasing cytotoxic T cell infiltration in the tumor microenvironment, decreasing regulatory T cells, stabilizing MHC class I expression on tumor cells, and modulating lipid metabolism. Early-phase clinical trials are ongoing for several cancer types, especially non-small cell lung cancer (NSCLC) and microsatellite stable/mismatch repair-proficient (MSS/pMMR) colorectal cancer, to determine if this combination improves responses in patients refractory to PD-1 blockade alone[1][3][5][6].
