PD-1 mesoCAR-T (also known as αPD-1-mesoCAR-T) is an investigational autologous chimeric antigen receptor (CAR) T-cell therapy designed to treat mesothelin-expressing solid tumors. The T cells are genetically engineered to express a CAR targeting mesothelin (MSLN) and to simultaneously secrete a full-length anti-PD-1 antibody or nanobody. This dual mechanism aims to directly eliminate MSLN-positive cancer cells while locally inhibiting the PD-1/PD-L1 checkpoint pathway to counteract immune suppression and prevent T-cell exhaustion within the tumor microenvironment. Developed by institutions including the Shanghai Tenth People's Hospital and Shanghai Cell Therapy Group, the therapy has been evaluated in Phase I clinical trials (e.g., NCT03615313) for patients with malignant pleural/peritoneal mesothelioma and advanced epithelial ovarian cancer.
