PD-1Ab21-BCMA CAR-T (CD203) is an autologous, next-generation chimeric antigen receptor T-cell (CAR-T) therapy engineered to target B-cell maturation antigen (BCMA). Beyond the standard CAR construct, these cells are modified to locally secrete a fusion protein consisting of a PD-1 antibody and interleukin-21 (IL-21). This multi-functional approach is designed to overcome the immunosuppressive tumor microenvironment of multiple myeloma; the PD-1 antibody provides localized checkpoint blockade to prevent T-cell exhaustion, while the IL-21 component acts as a cytokine support to enhance CAR-T cell expansion, persistence, and cytotoxic activity. Developed by Daihong Liu and researchers at the Chinese PLA General Hospital, the therapy is specifically being evaluated as a first-line consolidation treatment for multiple myeloma patients, including those with renal dysfunction who often face limited therapeutic options.
