Pefcalcitol is a synthetic vitamin D3 analog developed for topical use, primarily investigated for the treatment of psoriasis. It acts as an agonist of the vitamin D receptor (VDR), modulating gene expression to inhibit keratinocyte proliferation and promote differentiation in the skin. This mechanism helps reduce psoriatic plaque formation and inflammation. Pefcalcitol was designed to have high efficacy with reduced risk of hypercalcemia compared to other vitamin D analogs.
