PEG-PAM is a PEGylated recombinant form of the peptidylglycine alpha-amidating monooxygenase (PAM) enzyme, currently in preclinical development by PAM Theragnostics. The drug is designed as an enzyme replacement therapy to address neurovascular dysfunction and blood-brain barrier (BBB) deterioration associated with Alzheimer's disease and other forms of cognitive decline. PAM is the sole enzyme responsible for the C-terminal amidation of many signaling peptides, a modification essential for their biological activity. By restoring PAM activity, PEG-PAM aims to activate protective vascular peptide hormones, such as adrenomedullin, which help maintain vascular integrity and counteract BBB damage. It is intended for weekly subcutaneous self-administration.
