Pegadricase is a pegylated recombinant urate oxidase (uricase) enzyme developed for the treatment of chronic refractory gout. It functions by catalyzing the conversion of uric acid, which is poorly soluble and forms crystals in gout patients, into allantoin, a highly soluble metabolite that is easily excreted by the kidneys. Pegadricase is derived from a recombinant source and is modified with polyethylene glycol (PEG) to extend its circulating half-life and reduce its inherent immunogenicity. It is primarily utilized as the enzymatic component of the combination therapy SEL-212, where it is co-administered with ImmTOR (SVP-Rapamycin nanoparticles) to mitigate the formation of anti-drug antibodies (ADAs) that typically limit the long-term efficacy and safety of uricase-based treatments.
