Pegaptanib + ranibizumab is a combination of two anti-VEGF (vascular endothelial growth factor) agents used in the treatment of neovascular (wet) age-related macular degeneration (AMD). Pegaptanib is an aptamer that selectively binds to and inhibits the VEGF165 isoform, thereby reducing abnormal blood vessel growth and leakage in the retina. Ranibizumab is a monoclonal antibody fragment that binds all isoforms of VEGF-A, providing broader inhibition of VEGF-mediated angiogenesis. Both drugs are administered via intravitreal injection and have demonstrated efficacy in improving or maintaining visual acuity in patients with wet AMD. The rationale for combining these agents would be to potentially leverage their complementary mechanisms—pegaptanib’s selectivity for VEGF165 and ranibizumab’s pan-VEGF-A inhibition—to optimize therapeutic outcomes, although clinical evidence supporting routine combination use remains limited[1][2][5].
