Pegaptanib sodium is a pegylated RNA aptamer and selective vascular endothelial growth factor (VEGF) antagonist used for the treatment of neovascular (wet) age-related macular degeneration (AMD). It is a covalent conjugate of a 28-nucleotide oligonucleotide linked to two polyethylene glycol (PEG) units. Pegaptanib specifically binds to the VEGF165 isoform, inhibiting its interaction with VEGF receptors on vascular endothelial cells. This action blocks angiogenesis and reduces vascular permeability in the eye, thereby controlling abnormal blood vessel growth and leakage associated with wet AMD. Pegaptanib was originally discovered by NeXstar Pharmaceuticals (later merged with Gilead Sciences), further developed by Eyetech Pharmaceuticals/OSI Pharmaceuticals in the US, and marketed outside the US by Pfizer[1][3][4][6].
