This is an investigational multi-agent chemotherapy regimen combining five drugs—pegaspargase, gemcitabine, etoposide, liposomal mitoxantrone hydrochloride (Lipo-MIT), and dexamethasone. The combination is being studied primarily for the treatment of advanced or relapsed/refractory extranodal NK/T-cell lymphoma (ENKTCL) and associated hemophagocytic lymphohistiocytosis (HLH). Each component has a distinct mechanism of action: - Pegaspargase is a pegylated form of L-asparaginase that depletes asparagine, inhibiting protein synthesis in tumor cells. - Gemcitabine is a nucleoside analog that inhibits DNA synthesis. - Etoposide inhibits topoisomerase II, leading to DNA strand breaks. - Liposomal mitoxantrone hydrochloride intercalates into DNA and inhibits topoisomerase II; the liposomal formulation may improve delivery and reduce toxicity. - Dexamethasone is a synthetic glucocorticoid with anti-inflammatory and cytotoxic effects on lymphoid cells. The regimen aims to target cancer cells through multiple pathways to enhance efficacy in aggressive hematologic malignancies[1][2][3].
