Overview

The P-IE regimen is a combination chemotherapy protocol consisting of pegaspargase, ifosfamide, dexamethasone, and etoposide. It is primarily investigated and used for the treatment of extranodal natural killer/T-cell lymphoma (ENKTL), a rare and aggressive subtype of non-Hodgkin lymphoma. The regimen leverages the unique sensitivity of NK/T-cell lymphoma cells to L-asparaginase (delivered here as the long-acting pegaspargase), which depletes asparagine, an essential amino acid for these tumor cells. Ifosfamide acts as an alkylating agent, etoposide inhibits topoisomerase II to induce DNA breaks, and dexamethasone provides lymphocytolytic effects and reduces inflammation. This combination has been studied by institutions like Union Hospital, Tongji Medical College, to improve outcomes in patients with newly diagnosed or relapsed/refractory ENKTL.
