**pegenzileukin + pembrolizumab** is an investigational combination therapy consisting of **pegenzileukin (SAR444245)**, a pegylated engineered interleukin-2 cytokine agonist, and **pembrolizumab**, a PD-1 immune checkpoint inhibitor monoclonal antibody. Pegenzileukin acts as an IL-2 receptor agonist (selectively targeting immune-activating pathways while minimizing regulatory T cell activation), compared to native IL-2, designed to enhance antitumor immune responses. Pembrolizumab works by blocking the PD-1 receptor, preventing tumor-mediated inhibition of T cell activity, thereby restoring T cell–mediated antitumor immunity. This combination has been tested in phase 2 clinical trials across several advanced cancers, including gastrointestinal cancer, head and neck squamous cell carcinoma, and B cell lymphoma. Trials have been terminated or discontinued for some indications based on sponsor strategic decisions, not safety concerns.
