Combination of two recombinant hematopoietic growth factors used to stimulate myeloid recovery. Pegfilgrastim is a PEGylated granulocyte colony-stimulating factor (G-CSF) analog that binds the G-CSF receptor (CSF3R) to promote proliferation, differentiation, maturation, and activation of neutrophil precursors and mature neutrophils, with neutrophil-dependent clearance allowing once-per-cycle dosing in chemotherapy-induced neutropenia. Sargramostim is recombinant human granulocyte-macrophage colony-stimulating factor (GM-CSF) produced in yeast that binds the GM-CSF receptor (CSF2R) and signals via JAK2-STAT pathways to stimulate survival, proliferation, and differentiation of myeloid progenitors, increasing neutrophils, monocytes/macrophages, and dendritic cells, and activating mature myeloid cells. Clinically, G-CSF and GM-CSF are used for prevention of febrile neutropenia and for acceleration of hematopoietic recovery after chemotherapy or stem cell transplantation; GM-CSF also has broader immunomodulatory effects. This entry describes the concurrent or sequential co-administration of pegfilgrastim with sargramostim; there is no uniquely branded fixed-dose combination.
