Pegfilgrastim-mobilized allogeneic peripheral blood stem cells is an investigational cellular therapy being evaluated for the treatment of hematological malignancies. The approach involves the administration of pegfilgrastim (a PEGylated granulocyte colony-stimulating factor) to HLA-identical sibling donors to mobilize CD34+ hematopoietic stem and progenitor cells into the peripheral blood. These cells are then collected via apheresis and transplanted into the recipient. Unlike standard filgrastim-mobilized grafts, pegfilgrastim-mobilized grafts are designed to modulate the immune environment by promoting the development of tolerogenic antigen-presenting cells and regulatory T cells. This mechanism is intended to reduce the incidence and severity of acute graft-versus-host disease (aGVHD) while preserving the graft-versus-leukemia (GVL) effect, thereby improving overall transplant outcomes.
