Pegloticase + methotrexate

Pegloticase + methotrexate is a combination therapy used for the treatment of chronic gout in adults who have failed to normalize serum uric acid with conventional therapies. Pegloticase is a PEGylated recombinant uricase enzyme that catalyzes the oxidation of uric acid to allantoin, a more water-soluble compound, thereby reducing serum uric acid levels. Methotrexate is an immunomodulatory small molecule that inhibits enzymes involved in nucleotide synthesis, suppressing inflammation and immune responses. The addition of methotrexate to pegloticase therapy has been shown to increase response rates, reduce anti-drug antibody formation, and improve sustained lowering of serum urate levels compared to pegloticase alone[1][2][3][4][5][7][8]. This combination is particularly indicated for patients with uncontrolled or refractory gout.
