Peguricase is a PEGylated recombinant uricase (urate oxidase) enzyme developed by the Shanghai Institute of Biological Products for the treatment of gout, particularly in patients whose condition remains uncontrolled by conventional uric acid-lowering therapies. As a biologic therapy, it functions by converting uric acid into allantoin, a highly soluble metabolite that is easily excreted by the kidneys. The PEGylation of the enzyme is designed to extend its circulating half-life and potentially reduce immunogenicity compared to non-modified urate oxidase. It is currently being evaluated in Phase 1 clinical trials both as a monotherapy and in combination with methotrexate, where the latter is intended to further mitigate the formation of anti-drug antibodies and enhance treatment response rates in refractory gout cases.
