Pegydone is a PEGylated small molecule derivative of the immunomodulatory drug pirfenidone, currently under development by i-Cordis. Designed as a new molecular entity, Pegydone utilizes a small PEG chain conjugation to address the gastrointestinal tolerability issues and poor pharmacokinetics associated with the parent compound. It is primarily being investigated for its cardioprotective and anti-fibrotic effects in conditions such as heart failure with preserved ejection fraction (HFpEF), post-myocardial infarction heart failure, and muscular dystrophy-associated cardiomyopathy. Preclinical studies suggest that Pegydone offers superior therapeutic effects and reduced toxicity compared to pirfenidone, with potential applications in treating post-operative atrial fibrillation, immunotherapy-associated myocarditis, and idiopathic pulmonary fibrosis.
