Overview

Pegylated granulocyte colony-stimulating factor (G-CSF) is a long-acting biologic agent used as supportive care to stimulate the production of neutrophils and reduce the risk of febrile neutropenia in patients undergoing myelosuppressive chemotherapy. It is a pegylated form of recombinant human G-CSF (filgrastim), created by the covalent attachment of a polyethylene glycol (PEG) polymer. This modification increases the hydrodynamic size of the protein, thereby reducing renal clearance and significantly extending its plasma half-life compared to non-pegylated G-CSF. This allows for a single subcutaneous dose per chemotherapy cycle rather than daily injections. The drug acts as an agonist at the G-CSF receptor (CSF3R) on hematopoietic progenitor cells in the bone marrow, promoting their proliferation, differentiation, and activation into mature, functional neutrophils. Common examples include pegfilgrastim and lipegfilgrastim.
