Overview

Pegylated granulocyte colony-stimulating factor (PEG-G-CSF) is a long-acting formulation of recombinant human granulocyte colony-stimulating factor (G-CSF). It is produced by the covalent attachment of a polyethylene glycol (PEG) molecule to the G-CSF protein, a process that increases the molecule's hydrodynamic size and reduces its renal clearance. This modification significantly extends the circulating half-life of the drug, allowing for a single subcutaneous injection per chemotherapy cycle to provide sustained neutrophil support, in contrast to the daily dosing required for non-pegylated filgrastim. PEG-G-CSF acts as an agonist at the G-CSF receptor (G-CSFR) on hematopoietic cells in the bone marrow, stimulating the proliferation, differentiation, and activation of neutrophils. It is primarily indicated to decrease the incidence of infection, manifested by febrile neutropenia, in patients with non-myeloid malignancies receiving myelosuppressive anti-cancer drugs associated with a clinically significant risk of febrile neutropenia.
