This combination therapy, frequently referred to as the DDT regimen, consists of pegylated liposomal doxorubicin (Doxil), thalidomide, and dexamethasone. It was primarily evaluated in a Phase 2 clinical trial led by the University of Kansas Medical Center as an induction therapy for patients with newly diagnosed multiple myeloma who were candidates for high-dose chemotherapy and autologous stem cell transplantation. Pegylated liposomal doxorubicin is an anthracycline chemotherapy agent encapsulated in a polyethylene glycol (PEG) coated liposome, which extends its circulation half-life and reduces cardiotoxicity. Its mechanism involves DNA intercalation and the inhibition of topoisomerase II, leading to DNA strand breaks. Thalidomide is an immunomodulatory drug (IMiD) that binds to cereblon, part of an E3 ubiquitin ligase complex, resulting in the degradation of transcription factors essential for myeloma cell survival, while also providing anti-angiogenic effects. Dexamethasone is a potent synthetic glucocorticoid that induces apoptosis in lymphoid cells and acts synergistically with other anti-myeloma agents.
