PEGylated NELL-1 is a modified form of the osteogenic protein NELL-1 (NEL-like molecule-1) that has been conjugated with polyethylene glycol (PEG) to improve its pharmacokinetic properties. The PEGylation process significantly extends the protein's elimination half-life from 5.5 hours to 15.5 hours and increases distribution to bone tissues by 2-3 fold compared to unmodified NELL-1[2]. The most commonly studied form uses 5 kDa linear PEG (NELL-PEG-5k), which demonstrates optimal thermostability and bioactivity while maintaining the dual anabolic and antiresorptive properties of native NELL-1[2]. A further modified version, BP-NELL-PEG, incorporates bisphosphonate (alendronate) conjugation to enhance bone-targeting specificity[1]. This therapeutic protein promotes osteogenesis while suppressing osteoclast activity, making it a promising systemic treatment for osteoporosis and other bone-related disorders. The reduced injection frequency from every 2 days to every 4-7 days represents a significant improvement in clinical feasibility compared to naked NELL-1[2][7]. Developers include research teams from UCLA and collaborators from NASA/CASIS-sponsored research projects for treating spaceflight-induced bone loss[1].
