Pegylated recombinant human butyrylcholinesterase is a glycoprotein enzyme produced by recombinant methods and covalently conjugated with polyethylene glycol (PEG), significantly extending its plasma half-life and bioavailability compared to the unmodified protein[4][7][10]. The enzyme hydrolyzes acetylcholine and other choline esters, as well as many organophosphate nerve agents and drugs like cocaine. Its principal mechanism as a bioscavenger is to irreversibly bind and inactivate organophosphorus nerve agents, thereby preventing inhibition of endogenous acetylcholinesterase and neurological toxicity[2][4][7]. It has also been studied for cocaine toxicity protection. Protexia, the brand name product, has been developed primarily as a biodefense medical countermeasure for nerve agent exposure and entered clinical trials for this use[2][10].
