Pegylated recombinant human erythropoietin is a **recombinant protein** engineered by attaching polyethylene glycol (PEG) chains to human erythropoietin, which increases the molecule's size and prolongs its half-life in circulation. It acts by binding to the **erythropoietin receptor** on erythroid progenitor cells in the bone marrow, stimulating red blood cell production and preventing apoptosis of these cells. The longer-acting profile of PEGylated rHuEPO allows for reduced dosing frequency compared to standard recombinant erythropoietin. It is under investigation as a treatment for anemia in disorders such as chronic kidney disease and for reducing transfusion needs in other diseases characterized by impaired red blood cell production. PEGylation confers improved pharmacokinetics, reduced renal clearance, and high bioavailability. The drug distributes primarily to plasma, blood, bone marrow, and the injection site, with urinary elimination as the principal excretion route[1][3].
