Overview

Pegylated thymosin alpha 1 (SHR0401) is a long-acting immunomodulatory peptide developed by Jiangsu Hengrui Medicine for the treatment of chronic hepatitis B (CHB). It is a modified version of thymosin alpha 1 (thymalfasin), a synthetic 28-amino acid peptide identical to the natural peptide found in the thymus. By conjugating the peptide to a polyethylene glycol (PEG) moiety, the drug's half-life is significantly extended, allowing for less frequent dosing compared to the standard twice-weekly regimen of non-pegylated thymalfasin. The mechanism of action involves the activation of Toll-like receptors, specifically TLR9 and TLR2, on myeloid and plasmacytoid dendritic cells. This activation triggers the maturation of T-cells and the production of Th1-type cytokines, such as interferon-gamma and interleukin-2, thereby enhancing the host's cellular immune response against the hepatitis B virus. SHR0401 is currently being investigated in clinical trials to evaluate its efficacy in achieving HBsAg loss and functional cure in patients with chronic hepatitis B.
