Overview

This combination therapy consists of **peluntamig**, a first-in-class IgG-like bispecific antibody targeting DLL3 and CD47; **atezolizumab (Tecentriq)**, an anti-PD-L1 monoclonal antibody; **carboplatin**, a platinum-based chemotherapy agent; and **etoposide**, a topoisomerase II inhibitor. Peluntamig is developed by Phanes Therapeutics and acts by dual targeting DLL3 (a Notch pathway protein overexpressed on tumor cells) and CD47 (“don’t eat me” signal on cancer cells), aiming to enable immune recognition and attack of cancer cells. Atezolizumab blocks PD-L1, reactivating T-cells against tumors. Carboplatin forms DNA crosslinks leading to apoptosis, while etoposide inhibits DNA synthesis through topoisomerase II inhibition. The four-drug regimen is being evaluated for the treatment of extensive-stage small cell lung cancer, large cell neuroendocrine carcinoma of the lung, extrapulmonary neuroendocrine carcinomas, and treatment-emergent neuroendocrine prostate cancer in the SKYBRIDGE Phase I/II clinical trial[1][3][5][7].
