Pemafibrate (K-877) is a selective peroxisome proliferator-activated receptor alpha (PPARα) modulator (SPPARMα) developed for the treatment of dyslipidemia, specifically hypertriglyceridemia and mixed dyslipidemia. It works by activating PPARα to regulate lipid metabolism, lowering triglycerides and raising HDL cholesterol. Clopidogrel is an antiplatelet prodrug that inhibits the P2Y12 ADP receptor on platelets, thereby inhibiting platelet aggregation. The combination of pemafibrate and clopidogrel is not available as a fixed-dose product but may be used concomitantly in clinical scenarios involving cardiovascular prevention and dyslipidemia, especially in patients at risk for both atherothrombotic and metabolic disorders. No significant pharmacokinetic interactions are currently reported between pemafibrate and clopidogrel based on available data, but both drugs have CYP-mediated metabolism, requiring caution and monitoring. Clopidogrel was developed by Sanofi, and pemafibrate was developed by Kowa.
