This is a combination regimen of three agents: - **Pembrolizumab**: a humanized monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor, acting as an immune checkpoint inhibitor and used as immunotherapy for various cancers. - **Carboplatin**: a platinum-based chemotherapy that induces DNA crosslinking and cell apoptosis, used commonly in solid tumors. - **Ifinatamab deruxtecan (I-DXd)**: an investigational antibody-drug conjugate (ADC) targeting delta-like ligand 3 (DLL3), designed to deliver a topoisomerase I inhibitor payload specifically to DLL3-expressing tumor cells, under development for small cell lung cancer and other tumors. The molecule consists of a monoclonal antibody against DLL3 conjugated via a cleavable linker to a deruxtecan cytotoxic payload. The combination is under clinical investigation for lung cancer, particularly small cell lung cancer (SCLC) and non-small cell lung cancer (NSCLC)[4]. The combination leverages complementary mechanisms: immune activation (pembrolizumab), cytotoxic DNA damage (carboplatin), and targeted cytotoxic delivery to DLL3-positive tumor cells (ifinatamab deruxtecan).
