Pembrolizumab + gemcitabine + oxaliplatin is a combination therapy that brings together immunotherapy and chemotherapy for the treatment of various cancers, particularly biliary tract cancers (BTC) including intrahepatic cholangiocarcinoma (ICC). This combination leverages the immune-enhancing properties of pembrolizumab (a PD-1 inhibitor) with the cytotoxic effects of gemcitabine and oxaliplatin (GEMOX chemotherapy regimen). The combination works through multiple mechanisms: pembrolizumab blocks the interaction between PD-1 and its ligands (PD-L1 and PD-L2), enhancing T-cell activity against tumor cells, while gemcitabine and oxaliplatin directly damage cancer cells through DNA disruption. Notably, oxaliplatin in the GEMOX regimen has immunological effects that potentiate immune responses of tumor cells and synergize with PD-1 inhibitors like pembrolizumab.
