pembrolizumab + lenalidomide

Pembrolizumab + lenalidomide is an investigational combination therapy consisting of pembrolizumab, a monoclonal antibody that blocks the programmed death-1 (PD-1) receptor, and lenalidomide, an immunomodulatory small molecule (IMiD). Pembrolizumab acts as a checkpoint inhibitor to enhance anti-tumor immune responses by preventing tumor immune evasion via PD-1/PD-L1 pathway blockade. Lenalidomide has multiple mechanisms, including stimulating T cell activation, augmenting NK cell cytotoxicity, and inhibiting regulatory T cells that suppress anti-tumor immunity. This combination has been studied in clinical trials for relapsed/refractory multiple myeloma and other solid tumors, but pivotal phase III trials in newly diagnosed myeloma were halted due to increased risk of death and toxicity in the combination arm[2][3][5][6].
