Overview

Pembrolizumab + sargramostim is an investigational combination immunotherapy comprising a programmed cell death protein 1 (PD-1) checkpoint inhibitor and a recombinant human granulocyte-macrophage colony-stimulating factor (GM-CSF). Pembrolizumab is a humanized monoclonal antibody that binds to PD-1 on T cells, blocking its interaction with ligands PD-L1 and PD-L2, thereby restoring anti-tumor T-cell activity. Sargramostim is a yeast-derived recombinant GM-CSF that stimulates the production, differentiation, and activation of macrophages and dendritic cells. In the oncology setting, GM-CSF acts as an immune adjuvant by enhancing antigen presentation and the recruitment of tumor-infiltrating lymphocytes to the tumor microenvironment. This combination is designed to work synergistically: GM-CSF primes the immune system by promoting a more robust inflammatory environment and efficient antigen-presenting cell function, while pembrolizumab prevents the subsequent exhaustion of the activated T-cell population, aiming to overcome resistance to checkpoint inhibitor monotherapy in various solid tumors.
