Overview

This is a combination therapy consisting of three agents: pembrolizumab (a PD-1 immune checkpoint inhibitor monoclonal antibody), trastuzumab (a HER2-targeted monoclonal antibody), and tucatinib (a selective small molecule tyrosine kinase inhibitor targeting HER2). The combination is being investigated for the treatment of advanced or metastatic HER2-positive cancers, particularly breast cancer with brain metastases and other solid tumors that overexpress HER2. Pembrolizumab blocks the interaction between PD-1 and its ligands, enhancing T-cell mediated antitumor immunity. Trastuzumab binds to the extracellular domain of the HER2 receptor, inhibiting downstream signaling and mediating antibody-dependent cellular cytotoxicity. Tucatinib selectively inhibits the kinase activity of HER2, blocking proliferation signals in tumor cells overexpressing this receptor. Preclinical and early clinical data suggest that combining these agents may provide synergistic antitumor effects by both direct inhibition of tumor growth pathways and enhancement of immune-mediated tumor cell killing[1][6][10].
